This form is for reporting incidents of sexual harassment, sexual assault, dating/domestic violence, stalking, gender-based discrimination or other violations of Title IX. You may choose to identify yourself or remain anonymous.
This form is for non-emergency reports only. 

Anyone who experiences or becomes aware of an incident of Prohibited Conduct is strongly encouraged to submit a report to the New Lines Institute Title IX Coordinator. Reports can be submitted in the following ways:

    A Title IX Coordinator will review your report and reach out to discuss options, resources, and your rights. You control how much you participate. New Lines Institute will make every effort to maintain privacy and only share information on a need-to-know basis as allowed by law.
